Four-part bug report:
- What happened: "After submitting, the request does not appear in the staff list."
- What should happen: "It should appear at the top with status new."
- Steps to repeat: "Open form, fill all fields, add one photo, submit, open staff list."
- Evidence: the exact error message, or a screenshot.
Fix loop rule: if two attempts at the same fix fail, stop. Roll back, rewrite the bug report with more detail, or ask the builder to explain the cause before changing anything.
